Simply fill it out and call us with a credit card number of your choice. We will then open a file and have you send it to us via e-mail or fax (you can even mail it if you don't like computers!). Your case will be prepared in 10-15 business days or less.

By preparing in this fashion, we can be processing the case while you are assembling your documents, so no time is wasted.
